The Arnprior and District Family Health Team affirms their commitment to equal treatment of all our current and future patients respecting everybody’s individual choices, ethnicity, race, religion and circumstances. We stand with all communities in Ontario, Canada and around the world who are confronting and combating racism of all kinds.
We know that many Canadians face racism and discrimination and that it is a part of their daily reality. We commit to supporting our patients and the community to help those who experience this reality, through our work as health care professionals.
Who are we?
The Family Health Team is a distinct organization that supports and works closely with the 17 Family Physicians. The Family Health Team includes Nurse Practitioners, Registered Practical Nurses, a Pharmacist, Social Workers, a Dietician and a Respiratory Therapist to provide care five days a week, including supporting same day clinics for those requiring urgent care. These services are primarily available to patients who are rostered to a Physician, with some programs also offered to the general public.
Please visit our Programs & Services page to learn more about the programs we offer to patients of the Family Health Team as well as members of the community. For care closest to home, please connect with your Physician and the Family Health Team. If you have an urgent health emergency that cannot be immediately addressed, please refer to Health811 for more options.
We remain committed to working together to provide the best possible care for you and your family’s health needs.
Patient Experience Survey
The Arnprior Family Health Team is launching its 2023/24 Patient Experience Survey to gather your input and feedback. This information is analyzed and compared with other Family Health Teams across the region to understand how we are doing and look for ways to improve our delivery of health care services and better meet the needs of our patients. All data is aggregated, and responses are completely anonymous.
